Services Offered

Initial Psychiatric Evaluation

The Initial Psychiatric Evaluation typically involves a one hour Consultation. This varies based on the complexity of the patient’s situation. Typically, for children and teenagers, Dr. Attia may need 1-2 appointments to complete a full evaluation. After completion of the Initial Psychiatric Evaluation, Dr.
Attia will discuss her initial impressions, diagnoses, and treatment plan.

Follow Up Appointments

Once you/your Child has completed the Initial Psychiatric Evaluation, if necessary, Dr. Attia will recommend follow-up appointments, in accordance with her treatment plan. There are two types of follow-up appointments: A 20-25 minute session or a 45-minute session. In general, 20-25 minute follow up sessions are reserved for medication management, follow-up, and refills. The 45 minute sessions are for complex medication management or followup, psychoeducational sessions and/or psychotherapy sessions. Please be aware that, Dr. Attia requires that patients taking medication must be seen a minimum of every 3 months. If you have not had an appointment in 3 months Dr. Attia will not be able to fill your prescription.

Please ensure that you/your child are in a private, confidential setting during the appointment.

  • a legal guardian must be present for the whole scheduled session.
  • sessions will not be conducted in a moving vehicle and or in public places (i.e. parks, stores).

Before booking an appointment a credit card must be kept on file through intakeQ, Dr. Attia’s secure patient portal.

Dr. Attia is considered an “out-of-network provider” and does not participate in insurance plans. During your visit, you will be provided with a receipt
indicating the treatment and diagnosis code, which can be submitted to your insurance company for reimbursement. You will be responsible for contacting
your insurance company to determine reimbursement. For returned checks, a $30.00 fee will be charged for each returned check.

Dr. Attia charges for documentation and correspondence to schools (IEP/504 planning) as well as camp/school forms. Please allow at least 3 business days,
should you request a letter or form from Dr. Attia.

Office/Telemedicine Visit Fees:

  • One hour: $470
  • 45 min session: $365
  • 20-25 min follow up session: $235

Documentation Review:

Documentation review: including school/camp medication forms, 504/IEP support letters, reviewing IEP/ 504 or previous psychiatric records/psychological reports.

  • <10 min: $25
  • 11-20 min: $50
  • 21-30 min: $75
  • Basic school medication form: $15 per form

Two Business Day Cancellation Policy

The scheduled appointment time is set-aside specifically for you/your child. Dr. Attia kindly asks that you give two business days advanced notice for
cancellation or you will be charged Dr. Attia’s usual fee for the missed session.

***Example: If you have an appointment on a Monday, you would have to notify Dr. Attia by the prior Thursday to avoid a cancellation fee. If you or your child are ill, the appointment can easily be changed to a telemedicine visits.

A reminder email/text will be sent for appointments however this is a courtesy and patients are still responsible for attending appointments regardless if a reminder is received.

In case of an emergency, contact the local emergency room, crisis intervention services, or call 911.

Emails or Text Messages

Dr. Attia does not communicate via text messages but established patients can send a message in IntakeQ, the patient portal. Messages with be responded to within one business day.

Urgent or Emergency Issues

Dr. Attia will respond to voice/portal messages and phone calls as soon as possible; however, her practice does not support crisis or emergency services.
In the event of an emergency or urgent issue, which requires immediate assistance, please contact the local emergency room, crisis intervention services, or call 911.

Controlled Substance Policy

If you/your child is prescribed a controlled substance such as a psychostimulant or a benzodiazepine, please be aware that these medications can be easily abused and have addictive potential. For this reason, Dr. Attia requires that all patients who are prescribed controlled substance agree to the following:

  • If you are scheduling a psychiatric consultation with Dr. Attia and you have been prescribed a controlled substance by a previous provider, Dr. Attia may need to contact your previous provider before prescribing you a controlled substance.
  • If Dr. Attia does provide you with a prescription for a controlled substance, and there is concern that the medication is not working for you/your child or the dose is ineffective, please speak with Dr. Attia. Do not adjust the dose on your own.
  • Controlled substances are to be strictly taken as directed. Therefore, should a prescription be lost, stolen, or should you run out before you are due for a refill, the medication will not be refilled before you are due for a refill.

Prescription Refills

All refill requests should be made during your appointment time. Dr. Attia will give you enough medication to last you until your next visit. If you are about to run out and require a refill, Dr. Attia will require at least 48 hours notice to process this request.

For controlled substances, prescriptions are e-prescribed and Post dated scripts are sent in advance to your pharmacy. In order to get your next prescription, you have to request it through the pharmacy. Please contact your pharmacy before requesting a refill as this requires Dr. Attia to spend time
calling pharmacies that already have a prescription on file.

Confidentiality

Dr. Attia will release information only with your written permission unless otherwise required by law, including but not limited to:

  • Suspected abuse or neglect of a minor, elder, or dependent individual
  • A patient is in imminent danger of harming him or herself or another person
  • A patient communicates a serious threat of physical violence against another person
